How often should we be discussing bariatric surgery in our patients with obesity?

Always! I know we don't have a program here (yet), but we can facilitate referrals for our patients with insurance. Bariatric surgery - gastric sleeve surgery in particular - is one of the most effective, and cost-effective, treatments for obesity.
